Remote Clinical Documentation Review Specialist

24-MAG is seeking experienced professionals for a part-time consulting opportunity in clinical documentation review. This role focuses on analyzing structured clinical documentation, encounter notes, transitions of care, CDI queries, and EHR workflows.

Key Responsibilities

  • Encounter Documentation Review: Evaluate clinical documentation scenarios including SOAP notes, H&Ps, and progress notes against established standards. Identify documentation gaps and unsupported findings.
  • Transitions of Care & Medication Documentation: Review discharge summaries and medication reconciliation records against source materials and criteria. Prepare clear explanations for documentation decisions.
  • CDI Query & EHR Workflow Support: Assess CDI query scenarios and EHR documentation workflows (e.g., Epic, Cerner). Maintain accuracy and consistency in all submitted work.

Ideal Profile

  • 2+ years of experience in a relevant healthcare documentation role (medical scribe, CDI specialist, RN, NP, PA, etc.).
  • Experience with SOAP notes, H&Ps, discharge summaries, CDI queries, medication reconciliation, or EHR documentation.
  • Familiarity with EHR systems like Epic, Cerner, Meditech, etc.
  • Strong written communication skills and ability to explain clinical documentation reasoning.
