About this role
As a Primary Care Physician and Pod Leader, you will provide community-based care to vulnerable seniors, focusing on holistic and comprehensive patient care. Your role involves performing in-person assessments, developing personalized care plans with an interdisciplinary team, and coordinating patient discharge without hospital rounds, allowing you to see an average of 8 patients a day. What you need
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Master's or doctorate Degree
- Active state physician license without restrictions
- State CDS certification and DEA registration
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter
- Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants
Nice to have
- Board Certification preferred
- Experience in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics
- CPR and First Aid certification upon hire preferred
