What you need

  • MD or DO, board certified in Internal Medicine, Family Medicine, or Geriatrics
  • Active, unrestricted license to practice medicine in New Jersey
  • Current DEA registration in good standing
  • Eligible to enroll and bill as a Medicare practitioner
