Responsibilities
Clinical Care & Patient Identification
- Identify patients nearing the end of life across inpatient and outpatient settings.
- Deliver comprehensive therapeutic patient care aligned with evidence-based practices.
- Integrate patient values and preferences into individualized care plans.
Goals of Care Facilitation
- Coordinate and facilitate high-quality goals of care conversations with patients and families.
- Collaborate with inpatient and outpatient care teams to ensure aligned communication.
- Support decision-making regarding advance directives, CPR, intubation, and artificial nutrition.
- Guide discussions on preferred setting for end-of-life care.
Prognosis Communication & Care Coordination
- Partner with primary outpatient cancer providers and inpatient teams to reach consensus on prognosis.
- Communicate prognosis clearly and compassionately to patients and families.
- Coordinate care planning across inter-professional teams to ensure continuity.
Education, Support & Advocacy
- Educate patients and families on treatment options and expected outcomes.
- Provide emotional support to patients and caregivers during critical decision-making.
- Advocate for care plans that prioritize comfort and quality of life.
Documentation & Program Support
- Ensure accurate and timely documentation in the medical record.
- Participate in data collection to support program evaluation and quality initiatives.
- Collaborate with clinical and operational leadership to advance the Goal Concordant Care Initiative.
Qualifications
EDUCATION
- Required: Master's Degree Nursing or Doctorate Nursing Practice.
- Degree may be waived for applicants who obtained recognition as an Advanced Practice Registered Nurse from the Texas State Board of Nursing prior to 1/1/2003.
- Graduate education and related Texas Board of Nursing authorization must be consistent with clinical practice area and patient population.
