Band 6 Occupational Therapist – Intermediate Care (Community)

Location: Sheffield
Contract: Temporary

Our client is seeking an experienced Band 6 Occupational Therapist to join their Intermediate Care Team in the Sheffield area. This role involves working within the community to provide rehabilitation and support for patients transitioning from hospital to home, aiming to prevent readmissions and promote independence.

Key Responsibilities:

  • Deliver occupational therapy interventions within an intermediate care service, supporting patients in their own homes or community settings.
  • Complete functional assessments and develop individualized rehabilitation plans.
  • Work collaboratively with the multidisciplinary team to ensure safe and timely discharges.
  • Promote independence and optimize patient outcomes through evidence-based practice.

Requirements:

  • HCPC registration as an Occupational Therapist.
  • Experience in community rehabilitation or intermediate care.
  • Strong understanding of discharge pathways and admission avoidance strategies.
  • Excellent communication and teamwork skills.
  • A valid driving licence and access to a car (essential for community visits).
