What You Will Do
- Be a Catalyst for Change: Use your nursing expertise to connect mobile healthcare resources, integrating services and innovations to improve patient outcomes.
- Telephonic Patient Assessments: Assess patient needs remotely using evidence-based protocols, helping them navigate the complex healthcare system.
- Empower & Educate: Provide vital health education, support healthy behaviors, and help patients understand their care options across the healthcare continuum.
- Leverage Technology: Engage in telemonitoring activities, utilizing cutting-edge tools to track progress and intervene proactively.
- Lead & Collaborate: Play a key role in team care planning, mentoring colleagues, and championing new approaches in healthcare delivery.
- Promote Our Mission: Represent GMR’s innovative programs within the community, reaching populations that need care most but often go underserved.
- Facilitate & Educate: Serve as a facilitator and educator for GMR’s integrated healthcare training, empowering others to deliver better care.
- Deliver Excellence: Manage your time, resources, and documentation efficiently while providing outstanding customer service and support.

Minimum Required Qualifications
- Valid RN Compact State License.
- Minimum of 3+ years of RN experience in Emergency, Acute Care, or a similar fast-paced environment.
- Experience with clinical data collection, ethical principles, and patient advocacy.
- Graduate of an accredited nursing program.
- Current RN license in a compact state.
- Current CPR certification.
- Bachelor’s or master’s degree in healthcare administration, Public Health, or related fields is a plus, especially if focused on community health or disease education preferred.
