Job Summary
Administers optimal patient care by planning, organizing and implementing a physical therapy program including assessment, evaluation, documentation and treatment for individual patients referred by Licensed Independent Practitioner. Assists with review and development of department and program policies and procedures and with the supervision/education of other staff. All assessment and treatment media, approaches and techniques will be appropriate and specific to population served including general rehabilitation services to pediatric, adolescent, adult and geriatric patients.
Responsibilities
- Develops individualized treatment plan incorporating functional tasks appropriate for each patient after medical record review and patient evaluation
- Implements treatment plan in accordance with patient and multi-disciplinary team goals in a timely manner.
- Assesses patients equipment needs on an ongoing basis and provides equipment to ensure optimal positioning, functioning and/or mobility
- Provides modalities or other procedures in accordance with established protocols and documented competency with appropriate supervision
- Analyzes and applies appropriate functional limitation rating during evaluations and reassessments of all Medicare patients, considering the patient’s therapy CAP limitations and educating patients as needed
- Performs related duties as required.
Qualifications
- Required Education: Bachelor or Master’s degree from an accredited school of Physical Therapy.
- Required Experience: Must possess ability to work collaboratively with interdisciplinary team.
- Required Certifications/Registrations/Licenses: Must have current licensure in South Carolina. American Heart Association BLS for Healthcare Providers certification required, must be obtained within 7 days of hire.
