Position Summary
The Registered Nurse, Care Navigator is responsible for case management specifically for kidney health. This role involves coordinating care across the continuum to ensure high-quality, cost-effective outcomes for patients. The Care Navigator will focus on the whole patient and coordinate care delivery. This includes managing a caseload of members with varying acuity, entering member data into HealthMap’s Care Management documentation system (Compass), conducting health and social determinants of health screenings, performing medication reconciliation, creating and maintaining patient-centric care plans, updating identified problems, barriers, interventions, and goals, and assisting with ongoing case management. Collaboration with internal and external healthcare professionals is key to ensuring positive patient outcomes and coordinated care.
Responsibilities
- Handle in and outbound calls to provide world-class service to members.
- Educate members on kidney health, related co-morbid conditions, and optimizing renal replacement therapy by explaining dialysis and transplant options.
- Engage members into HealthMap’s Kidney Health Program.
- Follow up with members based on complexity and cadence as per policy.
- Act as a patient advocate to address and resolve concerns or barriers.
- Utilize community resources and programs in care planning.
- Serve as a liaison between the patient, their support network, treating physician, and other ancillary providers as part of an interdisciplinary care team to coordinate care, resolve nursing problems, and assist patients in meeting individualized goals.
- Notify providers of identified patient needs according to policy.
- Comply with HIPAA privacy laws and all other federal, state, and local regulations.
- Adhere to company-defined operational policies and procedures and security policies.
- Be accountable for individual metrics and key performance indicators.
- Navigate technical applications including Excel, OneNote, Outlook, and Word.
- Support after-hours and various time zones based on business needs.
- Drive patient and family engagement in their own care and support self-management.
Requirements
- Active, unrestricted RN license required.
- Bachelor’s degree required; however, 6+ years of RN experience including 3+ years in case management may be considered in lieu of a degree.
- CCM preferred.
- Three (3) years of experience in case management preferred.
- Experience in a dialysis center or transplant center preferred.
- Experience with Medicare and Medicaid preferred.
