About the role
Registered Practical Nurse Part Time - Brierwood Gardens
Reporting to the Director of Care (DOC) or Director of Health and Wellness (DHW), the Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.
Key Responsibilities
- Work as a member of the interprofessional team applying a person-centered approach to care.
- Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate and up to date.
- Implement and evaluate the effectiveness of care/service plans and make changes as required.
- Assist with scheduling and participate in resident's care conferences.
- Facilitate resident's move in, move out, and transfers in collaboration with DOC/DHW.
- Provide direct resident care and medication/treatment administration.
- Process orders and safely keep drugs and narcotics, administering them in compliance with all regulatory requirements.
- Facilitate and provide resident/family education.
- Plan, coordinate, manage & evaluate resident's care plan with the resident, physicians, nursing team, and other providers.
- Coordinate nursing team's resident care assignments for efficient service delivery.
- Respond to and manage significant changes in resident's condition and medical emergencies, including providing first aid.
- Teach, delegate to, and direct unregulated care providers.
- Respond to resident/family concerns and ensure appropriate action or communication.
- Follow and assist DOC/DHW in coordinating and monitoring Infection Prevention and Control, Health and Safety, and Clinical Programs.
- Uphold and promote the organization's values and philosophy, particularly relating to ethics, morality, and integrity.
- Complete all other duties as assigned.
Qualifications
- Current registration as a Registered Practical Nurse in the applicable Province/State.
- Experience working in a seniors' environment and with unregulated care providers is preferred.
- Education and experience in gerontology, rehabilitation, community/complex/long term care is an asset.
- Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills.
- Demonstrated sound clinical knowledge.
- Current Basic Cardiac Life Support and First Aid certification required.
- Computer proficiency and experience with Microsoft Office applications.
- Current (within 6 months) Vulnerable Position/Sector Screening (VPS).
