Schedule: Full-time Monday–Friday
Location: Field Based (Baytown and surrounding areas)
Compensation: $75.00 per productivity point (with expectation of 30 points/week)

As a Physical Therapist, you are central to this mission, playing a vital role in restoring patient function, ensuring their safety at home, and achieving tangible outcomes. We also leverage technology, including a proprietary charting product to reduce documentation burden and an upcoming scheduling product designed to simplify routing and visit coordination, thereby preserving your valuable time.
What You Will Do
- Manage your caseload as a case manager, taking ownership of the patient’s therapy plan, progression, and overall outcomes.
- Conduct comprehensive in-home assessments and deliver skilled Physical Therapy visits.
- Develop and implement clear plans of care focusing on gait/balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
- Establish measurable goals, monitor objective progress, and adapt treatments promptly when patients encounter challenges.
- Coordinate with the multidisciplinary care team, communicating condition changes, aligning on orders, and ensuring cohesive patient care.
- Directly manage treatment and outcomes, including PTA supervision when necessary.
- Educate patients and caregivers on practical, implementable home programs.
- Document patient care promptly and thoroughly within Homecare Homebase (HCHB), ensuring documentation is defensible, efficient, and consistent.

Requirements
Must-haves:
- Active Texas PT license in good standing.
- Strong clinical judgment and ability to work independently in the field.
- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
- Excellent communication and documentation skills.
- Reliable follow-through on orders, frequencies, and patient goals.
- Current driver’s license and comfort with daily travel within the assigned territory.
- Ability to carry/transport basic therapy equipment as needed.
- Proficiency in core home health PT scope, including safety-first mobility work (transfers, gait training, balance/fall prevention, assistive device recommendations).
- Expertise in strengthening, functional mobility, and home safety assessment/modification education.
- Skilled in patient/caregiver coaching with practical home programs.
Nice-to-haves:
- Prior home health experience.

- Deeper clinical experience in high-signal PT populations.
