- Doctor of Pharmacy (PharmD)
- Licensed pharmacist in Washington
- Completion of a PGY2 Ambulatory Care Residency or PGY1 Residency with 1 year of ambulatory practice under a CPA or at least 3 years of ambulatory practice with disease state management
