About this role
The MDS Nurse Transition Leader at Otterbein will take on MDS Nurse duties at various locations as needed, ensuring effective completion of MDS and care plans, coaching staff, and managing discharge planning. When not filling an interim role, the leader will work on special projects assigned by the VP of Clinical Quality, contributing to the development of new processes and participating in team meetings.

What you need
- BSN
- Current OH RN license
- Minimum 5 years of MDS experience in long-term care/nursing home environment
- Proven track record of strong MDS skills (casemix management, PDPM management, Quality Measures)
- Experience with Electronic Medical Records; PCC experience highly preferred
- Detail-oriented with excellent literacy, writing, documentation, communication, and interpersonal skills
