About this role
The Senior Care Integration Nurse (RN) will provide direct Enhanced Care Management (ECM), Community Supports (CS), and transitions of care (TOC) support for Alliance members. Responsibilities include monitoring provider performance, coordinating care across teams, and assisting with program implementation and compliance. This is a full-time temporary position lasting from September 2026 to December 2026, with a focus on community-based nursing and care management.
